The membership card is recognized within Fullerton hospital providers, so please always carry your Membership Card just in case you require medical assistant or treatment.
Hospital sometimes asks an authorization from the patient or his family before releasing the medical information to Aetna.
Hospital is required to complete a PCMF outlining details of the medical condition and treatment to be undertaken.
Aetna needs this information to determine if the treatment is eligible.
If treatment is eligible, Aetna will then offer direct settlement to the hospital.
Please ensure that the hospital confirms with you that this has been sent to Aetna.
Aetna will inform you if the treatment is eligible under the terms of the plan.
In case of reimbursement, you and your treating doctor need to complete this form and submit along with the other claim documents.

Just in case you have not registered your secure member website that gives you tools to manage your health benefits. Please register in just a few steps by visiting www.aetnainternational.com and clicking “Member” under the “Secure login” section. You will need to enter your name, date of birth, and the ID number on your Member ID Card.
